Searching for Text

Hello,
        I am working on a project and I need a text that so far I have been unable
to obtain. The text is the following:

Chrysostom Baptismal Instructions
Papadopoulos-Kerameus 3, paragraphs 11 - 15.

        An English translation is available in
        St. John Chrysostom: Baptismal Instructions. Ancient Christian Writers,
vol. 31 (Westminster, Md. and London 1963), pgs. 164 - 165.

        The above is only English; I need the Greek text. I know the source for
the Greek but have not had any luck in obtaining it. Here are the two
sources for it:

codex gracecus 215 located in the Synodal Library of Moscow

and in a book

Varia graeca sacra. Sbornik greceskikh neisdannikh bogoslovskikh tekstov I
- IV vekon (St. Petersburg 1909).

        In the book just previously mentioned the text is located someplace
between pages 154 and 183. There are three American Universities that have
a copy (Chicago, Harvard, and Illinois). I am basing this information off
the footnote in the Harkins book. Needless to say I have not had luck with
Interlibrary loan.
        If anyone has access to the book mentioned would it be possible to have
the one or two pages photocopied that I need or (scanned and e-mailed),
etc. If anyone can assist me please write back.
