The old (1976) USDA Agri. Handbook No. 496, Insect Pollination
of Cultivated Crop Plants, by S. E. McGregor has a section on
constructing a bumble bee nesting box (incl. pictures).

Also, Insect Pollination of Crops (1970) by J. B. Free, Academic
Press, has instructions on same.

Also, a periodical, Bumblebeequest, comes out of Canada.  Contact
