Help with postal addresses of NT Papyri holders

Dear everyone,

Please help me fill in the blanks in the following _postal_ addresses, if 
you have them handy! Just email them straight to me.

I am interested in getting high quality digital images of the NT MSS onto
CD-ROM starting with the papyri, so I want to write to the holders to ask
for their permission to use the papyri in their collections. For this
reason I am trying to find the addresses of the institutions which hold
the NT papyri. I've looked at NA27, Aland's _Kirzgefastliste..._ and the
1994 _World of Learning_ to get what I have so far, but I do not have
complete addresses for the following:

Thank you for your help. By the way, what do you think of the idea? It may
be that the image capture could be greatly facilitated by the use of
Internet. For example, the holding institutions could take the digital
pictures using one of the high resolution digital cameras now available
(_not a video camera_), and sent the resulting file (about 10 Megs per A4
size sheet) to the publisher via the network.
